The stages of a pressure injury are: Stage 1: The area of skin is inflamed, red, purple, or blue. The skin is intact with an area of non-pure erythema and warmth over the bony prominence. Stage 2: Damage to the skin or skin cells can cause skin infections. It looks like an open, red, moist sore. There is no visible fat and deep tissue. Stage 3: Thick skin disappears, fat becomes visible, and lean tissue becomes visible. Muscles, tendons, or bones are not visible to the outside environment. A pressure fracture causes a deep cavity. Stage 4: The skin has broken down and the surrounding tissue begins to necrosis, which can affect muscles, bones and joints. Unstagable: a full-thickness pressure injury covered by debris and scarring.
